Insurance
We accept all PPO plans except Cigna & HMO plans. We are in network with BCBS, Aetna, PHCS, UHC, & Medicare. If you do not see your insurance plan listed please call our office as we are always updating our system. For your convenience, we will call and verify your insurance benefits prior to your first visit.
FAQs
If you are a new patient, please arrive 15 minutes early to your appointment to fill out necessary paperwork. For your convenience, you may also print out the forms from our website and bring them with you to your appointment. Please be sure to bring your insurance card and driver’s license or photo ID. If you have had past imaging ( MRI, X-ray, CT scan), please bring those films with you or have them sent over prior to your appointment.

If you do not have insurance or your insurance does not cover the full cost of your treatment, we offer a variety of payment options to meet your needs. Please call our billing office to discuss payment arrangements prior to your appointment.
